How To Volunteer at Ohio’s Community Mercy Hospice

We are excited that you have decided to volunteer and serve our patients and families. The Volunteer Services Department is available to assist you.

Step 1 – Willingness to support our patients and families

  • Download the Volunteer Application to your computer and print.

  • Download the Volunteer Reference form to your computer and print.

  • E-mail or give the printed Volunteer Reference form to two people NOT related to you and have them complete the reference within one week of the date you gave it to them.
  • SIGNATURE MUST BE INCLUDED

Fill out both the Volunteer Application and the Volunteer Reference form COMPLETELY and send them to:
